CAPTA is an initiative to assist Lee County families who are affected by substance abuse. Nurse Home Visitors provide a host of services and resource assistance for pregnant women, new mothers and fathers, and their children up to one year of age.
By building on the strengths you have as an individual, within a family, and within your community, together we can reduce the risk associated with infants who may have been affected by prenatal exposure to alcohol or drugs.

After submitting this form, a Connect coordinator will contact you and help you determine which Home Visiting program fits your needs best. The Home Visitor will schedule regular visits based on your goals.

All of our programs are powered by Connect, Florida’s coordinated intake and referral process. Connect is the one-stop entry point to connect families with home visiting programs and other resources to improve outcomes for moms and babies.
