Healthy Start Home Visiting is a free and voluntary program that supports families during pregnancy and after the baby is born. A trained professional like a nurse, social worker, or health educator visits your home to provide helpful information and support for you and your baby. Your Home Visitor will work with you one-on-one to answer questions, share tips on caring for yourself and your baby, and connect you with resources in your community. This is an educational program designed to help you feel confident and prepared as a parent. This program is available through Connect, Florida’s coordinated system that helps families find the right support services. Connect is a one-stop resource that links families to home visiting programs and other tools to help moms and babies thrive.

After submitting this form, a Connect coordinator will contact you and help you determine which Home Visiting program fits your needs best. The Home Visitor will schedule regular visits based on your goals.
